Registrations Materials Required:
1. Original Roster from current 2011/2012 year (US Club Soccer or USYSA Roster) (Seal & signature required)
2. 2 copies of original roster with full guest player information written on the roster.
3. All player passes
4. Medical Release forms for all players (your league forms will suffice) and they do not need to be notarized.
5. Physician Release Note for any player with a hard cast (2 copies).
6. Permission to Travel form if traveling from outside Region 1.
